Life can be challenging, especially when you or someone you love is experiencing mental health concerns. At La Familia Central Valley, we provide compassionate, culturally responsive behavioral health services for children, adults, and families throughout the Central Valley.
Our bilingual team offers counseling, case management, Promotor(a) services, and other support in community settings, schools, and through mobile outreach. Through partnerships with counties, health plans, schools, and community organizations, we work to make quality behavioral health services more accessible throughout Stanislaus, Merced, and Madera Counties.

Outpatient Counseling Services:
Outpatient Counseling Services provide compassionate, supportive mental health care for individuals of all ages. Our therapists work with clients experiencing stress, anxiety, emotional challenges, or other difficult life circumstances, providing a safe and supportive space to talk, heal, and build healthy coping skills.
Whether you are experiencing stress, anxiety, emotional challenges, or difficult life circumstances, our team is here to help. Our bilingual therapists provide services in English and Spanish and work with individuals to develop goals and strategies that meet their unique needs.
Eligibility: Individuals of all ages who have Health Plan of San Joaquin or Central California Alliance for Health coverage.

Mobile Mental Health Program
The Mobile Mental Health Program brings mental health awareness and services directly to rural and underserved communities throughout Stanislaus and Merced Counties. Our mobile mental health vans help reduce transportation barriers and make it easier for community members to access behavioral health support within their communities.
Services include individual therapy, support groups, case management, and mental health screenings, providing individuals with convenient access to services based on their needs.

Eligibility: Residents of all ages in Stanislaus and Merced Counties who have Health Plan of San Joaquin or Central California Alliance for Health insurance. Thanks to support from Sutter Health, services are available at no cost, and all community members are welcome, regardless of insurance status.
Specialty Mental Health Services
La Familia Central Valley provides bilingual Specialty Mental Health services to eligible individuals with Serious Mental Illness (SMI) in Merced and Madera Counties. This program expands access to behavioral health care by connecting clients with qualified mental health professionals through convenient, secure telehealth services.
Services are designed to provide timely access to mental health care, reduce barriers to treatment, and connect clients with ongoing support and resources based on their individual needs. Telehealth appointments allow clients to receive services remotely, helping ensure continuity of care and improved access to behavioral health support.
Eligibility: Available to adults currently receiving Merced County Medi-Cal benefits who qualify for specialty mental health services.
Referrals: This program only accepts direct referrals from Merced and Madera County BHRS.
Enhanced Care Management
ECM offers extra services at no cost to Medi-Cal members who have complex needs and challenges that make it hard to improve their health. La Familia’s team works individually with each member and their support system to help establish a care plan that addresses all aspects of a member’s health and well-being and supports them on their journey to self-reliance.
Eligibility: Individuals must be enrolled in a qualifying health plan and meet the plan’s eligibility criteria for Enhanced Care Management (ECM) services. We are currently accepting the following Medi-Cal Managed Care Plans: Health Plan of San Joaquin, Health Net, Central California Alliance for Health, and Kaiser.
Referrals: Submit a Referral here to request ECM services for yourself or someone else. Our team will follow up with you and your health plan to determine whether you qualify for ECM services. You can also contact your health plan directly to request a referral or learn more about eligibility.
Full Service Partnership
The Full-Service Partnership (FSP) provides intensive, individualized mental health services to adults with Serious Mental Illness (SMI) in Merced County, including individuals who may be involved with the justice system. The program uses Assertive Community Treatment (ACT) and Forensic Assertive Community Treatment (FACT) models to provide comprehensive, community-based support tailored to each individual’s needs.
A multidisciplinary team works closely with each client to develop individualized goals, strengthen daily living skills, promote stability, and support greater independence. The team includes clinicians, mental health specialists, peer support specialists, a substance use disorder counselor, and administrative and supervisory staff.
The program takes a whole-person, team-based approach, providing individualized support and connecting clients with the services and resources needed to promote long-term wellness and successful community living. Eligibility: Adults receiving Merced County Medi-Cal benefits who qualify for specialty mental health services and meet ACT/FACT criteria.
Referrals: This program only accepts direct referrals from Merced County BHRS.
School Based Mental Health Services
Mental health counseling, case management, and group services for students and their families. Staff Therapists and Promotor(a)s are integrated into local school districts throughout Stanislaus & Merced Counties in order to provide necessary mental health services directly to students and their families. Helping students when they first need it, right where they are.
La Familia Central Valley currently provides school-based services in the following school districts:
Patterson Joint Unified School District
Winton Unified School District
Riverbank Unified School District
Hilmar Unified School District
Eligibility: Students ages 12+ in schools where La Familia has a presence. Services can be requested through your school counselor or administrator.
Referrals: Contact your school counselor or school administrator to request information or a referral.
Stan Connect
Our dedicated team of mental health professionals provides support services, all across Stanislaus County. Whether you’re going through a tough time or just need a little extra help, we’re here to support you and your family—at no cost. Now serving West Modesto, Airport District, and Riverbank. Eligibility: This program accepts students ages 12+ in schools where La Familia has a presence. Services can be requested through your school counselor or administrator.

We Care Program
La Familia Central Valley provides Substance Use Disorder (SUD) services to individuals experiencing homelessness in Stanislaus County through the We Care Program in Turlock.
Our team provides a welcoming and supportive environment where individuals can access counseling and recovery-focused group services, whether or not they are staying at the We Care Shelter. Eligibility: This program is available to any homeless person in Stanislaus County who is experiencing Substance Use Disorder (SUD).

Prodigal Sons and Daughters
Prodigal Sons & Daughters provides substance use prevention, intervention, and recovery support services for individuals and families in the community. We provide clients with access to supportive services designed to promote recovery, strengthen coping skills, and build connections within the community.
Services include individual counseling, recovery and support groups, life skills classes, learning groups, community connections, and youth substance use prevention and intervention programs. Youth services also include drug and alcohol education, vaping and tobacco intervention, and support focused on healthy decision-making and wellness.
